This website provides general program information. It is not the approved clinical intake system, patient portal, or emergency communication channel.
What this website should not collect
Do not send or enter diagnoses, symptoms, medications, insurance information, treatment records, custody documents, court records, Social Security numbers, or other private health information through ordinary website email links.
Clinical privacy
Before clinical operations begin, the responsible clinical entity must publish its Notice of Privacy Practices, 42 CFR Part 2 notice and consent architecture where applicable, release-of-information process, breach procedures, patient-access and amendment processes, record-retention standards, and contact information for the privacy officer.
Analytics and cookies
This standalone site does not include advertising trackers, analytics services, or third-party contact forms. Any future analytics, scheduling, chat, portal, or form tool requires privacy, security, accessibility, business-associate, data-flow, retention, and consent review before installation.
Virtual information—not a virtual intake.
The virtual pages contain program descriptions, pathway filters, example-day filters, and a print control. These features do not collect or store health information, assess eligibility, create accounts, or submit referrals.
