Reach One Clinical Services and the youth clinical continuum

The intended relationship positions Reach One Clinical Services as the clinical provider for youth assessment, treatment planning, Day Treatment, IOP, OP, clinical supervision, records, authorization, billing, and quality—while One Pulse provides the Cincinnati public identity, family and community connection, partnership development, and separately governed programming.

Potential role: youth sports, movement, mentoring, leadership, family engagement, and place-based trust across Reading, Lockland, Lincoln Heights, Woodlawn, and Wyoming.

Coalition of Dads

Potential role: father and caregiver engagement, approved mentoring, family activities, positive male presence, community tables, and family reinforcement.

Schools

Potential role: student-support discovery, referral partnership, facility approval, family communication, educational coordination, and school-specific data and safety agreements.

Older-adult network

The 55+ program should connect to systems already carrying the weight.

Council on Aging

Information, assessment, home- and community-based supports, caregiver resources, transportation, benefits, elder safety, and aging-network coordination.

Health systems and primary care

Referral, medication and chronic-disease coordination, discharge transitions, crisis escalation, and whole-person care.

Trusted community sites

Senior centers, libraries, churches, housing communities, meal sites, pharmacies, and home-health partners can reduce stigma and access barriers.
