Cognitive Renovations initiative

Reentry is a transition—not a single appointment.

The Cognitive Reintegration Initiative (CRI) is described in Cognitive Renovations’ materials as a bridge for people returning to Hamilton County. Its focus is reducing the practical barriers between release and a stable next step.

Plan the return before the release.

The supplied services page describes beginning engagement 30 days before release. The home materials describe “meet-at-the-gate” transportation and immediate reintegration planning. Those are partner-described program elements, not transportation or placement guarantees from this website.

Ask Wali and the Cognitive Renovations team what coordination is possible for the person’s release date, destination, and practical needs.

A conversation guide for the first contact

Start with what has to happen next.

Arrival: What date and destination are confirmed, and who is responsible for the handoff?

Immediate needs: What identification, transportation, food, communication, or housing questions need attention?

Connections: Which community supports or authorized providers need to be contacted?

Follow-up: What is the next action, who owns it, and when will it be checked?

Choose the appropriate channel. Do not send release records, court documents, diagnoses, participant histories, or identification numbers through ordinary website messages. Ask for the responsible organization’s approved private coordination process first.

03

Agree on the action.

Set a named contact and a specific follow-up date.

04

Close the loop.

Confirm acceptance, scheduling, completion, or the next barrier.
