My Peer Plan

Use this tool to clarify direction, map support, check BRAVE skills, create a 72-hour action plan, and print or export a personal summary. Your entries are not submitted to either organization by this app.

Before you begin: The Peer Plan is a browser-local personal organizer, not an EHR, official service record, clinical assessment, or emergency-response tool. Neither organization receives your entries through this tool. Do not use it as the only place for urgent safety information. For immediate danger call 911; call or text 988 for suicide or emotional crisis support in the United States.

My Direction

Describe recovery in your language. This is not a clinical assessment.

A personal tool is not a substitute for relationship.

Bring the parts you choose into a conversation with a peer supporter, clinician, family member, sponsor, mentor, or other trusted support. Qualified professionals remain responsible for clinical assessment, treatment decisions, safety planning, and medical advice.
