The personal Peer Plan

The Peer Plan starts in temporary page memory. Device storage is used only after you choose Remember this plan on this device or confirm Save locally. Entries are not submitted to either organization by this application. You may export a file, print a summary, or clear the saved copy.

Browser-local does not mean encrypted clinical storage. Other scripts operating on the same website origin may be able to access local storage. Avoid names, diagnoses, identification numbers, or sensitive records; use brief personal planning notes instead. Do not use it on a shared device for private information.

Clearing site data, changing the website domain, private browsing, or device loss may remove the plan. Exported files and printouts are your responsibility.

Public inquiry collection

This edition does not collect online inquiries. The website does not collect a contact-form submission. Call or email the appropriate organization with a general question instead.

No public message field should be used for protected health information or emergency concerns. Ask for the responsible organization’s approved private process first.

Separate organizations

Reach One Clinical Services and Cognitive Renovations remain separate organizations. The responsible provider, staff role, service scope, consent, records, and any payment arrangement are clarified before services begin.

Contacting one organization does not itself authorize that organization to disclose information to the other. Any later information exchange should follow the applicable consent and approved coordination process.

Cookies, logs & outside services

This module includes no advertising pixels, external analytics scripts, embedded maps, external fonts, or third-party photo requests. It loads its images and styling locally. Hosting systems may still maintain ordinary request and security logs.

This edition does not start PHP sessions or issue application session cookies. It has no contact-form collection endpoint; attempted submissions are rejected. Your broader website or host may use additional technologies outside this module.

External links and your phone or email applications follow their own privacy practices. The old offline page cache is retired; this module does not offer offline clinical storage.
