This application stores only fields explicitly marked for deidentified program planning. Those values remain in local browser storage on the device being used. Reach One does not receive or monitor those entries.
Program goals, role titles, implementation dates, population-level needs, partner categories, budget assumptions, readiness ratings, and deidentified planning notes.
Names, initials, dates of birth, Medicaid or insurance identifiers, exact participant statements, diagnoses, medication details, substance-use histories, safety narratives, staff-performance details, incident facts, signatures, or protected health information.
Participant plans, pre/post evaluations, placement decisions, life-safety incidents, consent and assent, ROI/authorization, and person-level handoff records remain excluded from local storage and export.
The Export button creates a JSON file containing only allowed planning keys. The Clear button removes the application’s local planning keys after confirmation.
Use the organization’s approved EHR, incident, consent, HR, contract, or secure case-management platform for official records.
