Who it’s for
- Adults 18+ with SUD recovery instability and functional barriers (housing, employment, legal, benefits).
- Common referrals: detox/residential step-down, courts/probation, hospitals/ED, partner providers, self-referral.
What we deliver
- CPST: housing applications, IDs, benefits, transportation, employment readiness, legal compliance planning.
- Peer Support: routine building, meeting linkage, barrier problem-solving, show-up support.
- Linkage to OP/IOP counseling when indicated.
Dosage & schedule (typical)
Final schedules and tracks will be published at activation.
- Typical start: CPST weekly + peer weekly (increase early, taper as stability grows).
- Typical episode: 8-20 weeks (acuity dependent).
Admission & exclusions (plain language)
- Good fit: needs structured follow-through support and coordination to maintain recovery and functioning.
- Not a fit: medical instability or need for residential monitoring/containment.
Care coordination
- Partners: housing providers, JFS, MAT prescribers, employers/training, probation, recovery orgs.
- Closed-loop: outcome is documented (scheduled/denied/waitlist/next step).
- ROI posture: verify ROI at each coordination touch.
