Partners interface with Reach One through defined lanes. Each lane has a predictable handoff pattern, documentation posture, and ROI requirements.
Schools (youth)
Common needs: attendance instability, behavioral dysregulation, IEP/504 coordination, safe after-school structure.
Closed-loop output: What we verify: engagement status, coordination outcomes, and next actions (with ROI).
Courts / probation
Common needs: compliance support, stability planning, barrier reduction, reliable attendance.
Closed-loop output: What we verify: attendance and coordination outcomes (with ROI or lawful authority).
Hospitals / ED
Common needs: step-down planning, rapid connection, relapse-risk stabilization.
Closed-loop output: What we verify: scheduled start, contact outcome, and step-down plan.
MAT / primary care

Closed-loop handoff standard
We do not treat “referred” as a completed action. The loop closes only when the outcome is verified and next steps are documented.
- Predictable acknowledgement of referral receipt and next step.
- Clear documentation of outcomes within ROI limits (minimum necessary).
- Transparent communication when the referral is not a fit, including alternatives.
ROI & privacy posture
Coordination is built around lawful authorization and minimum necessary disclosure. This protects clients and protects partners.
Release of Information (ROI)
- We do not coordinate with schools, courts, employers, or other partners without an active ROI, except where required by law.
- Each coordination touch verifies ROI status and documents the minimum necessary information.
- ROI scope is time-limited and purpose-specific.
SUD confidentiality note
When substance use disorder information is involved, additional confidentiality rules may apply (e.g., 42 CFR Part 2). Reach One’s posture is to use the strictest applicable standard and document the rationale.
Referral readiness checklist (non-PHI)
This is a pre-launch list of what typically makes a referral clean and actionable once the program is activated.
- Best contact info and safe way to reach the person/caregiver.
- Referral purpose in functional terms (what is not working).
- Any deadline constraints (court dates, school meeting dates) — without PHI in webforms.
- ROI plan: who can be contacted, and for what purpose.
- Clear confirmation of receipt and next step.
- Closed-loop outcome verification (connected/waitlist/redirected).
- Program routing guidance (E1-E6) based on level-of-need.
- Ongoing coordination within ROI limits.
